Same day or next day appointment
requests will be considered, based upon availability.
When arriving for an appointment,
please bring your insurance forms with all identifying
information. New patients are always welcome.

Dental Insurance
Our
office participates with most dental insurance plans.
Patients who assign benefits to the office are responsible
for their deductible and co-payment. Patients may also
have the option of paying the dental fees and then receiving
their insurance reimbursement check directly from the
insurance company. Check with your carrier. Please bring
your insurance identification card when you visit our
office.

Cancellations
If you are unable
to keep an appointment, we ask that you kindly provide
us with at least 24 hours notice. This will make
it possible to give your appointment to another patient.

Financial Policy
Payment is expected at the
time of each treatment. Co-payments and co-insurances
are due at the time services are rendered. Forms of payment
accepted by the office are checks, cash, insurance assignment
and the following credit cards: MasterCard, Visa, Discover,
American Express and Care Credit.

Telephone Calls
If you have questions which
only the dentist can answer, a message will be taken and
your medical file will be given to Dr. Veiga or another
staff dentist who will return your call.
If your question pertains
to an administrative matter, a qualified staff member
will be pleased to assist you.
